I've seen this happening about once a month when using the default nvidia driver. Switching to the nouveau driver cured it completely. Do you have any symptoms that could indicate a driver problem? Artifacts etc.? Henry

This works as long as the window isn't maximized.

I was going to ask how the desktop effects are set at. I've seen strange things happen when using OpenGL where the max, min and close disappear. Reverting back to xrender fixed the problem. Again this has to do with the nvidia drivers as Henry stated, and if things were upgraded lately, maybe this is something to look at.
